Royal Darwin hospital concedes patient’s death ‘avoidable’ as coronial inquest hears of difficulties in caring for mental health patients in general wards

The death of an Indigenous mental health patient at Royal Darwin hospital last year was avoidable and due to “a litany of failures”, a coronial inquest has been told

Ngalarina, a sistergirl also known as Xysc after changing their name in 2017, was an involuntary mental health patient at the hospital in 2022 when they died alone in a bathroom from volatile substance abuse.

The Northern Territory coroner asked media to use Ngalarina and they/them pronouns in line with their family’s wishes
